Varied Sources for Community Activities


1. Health Coverage Sources
Fill out a UIHS screening form to see if you qualify for one of the following services:

  • Healthy Families & Medi-Cal for Children
    Qualifying American Indian children up to 19 years of age may be eligible for free medical, dental, vision, and pharmacy coverage. To see if your children are eligible, click the following link to view the family size and income eligibility table.

  • Access for Infants & Mother (AIM)
    This is insurance coverage for pregnancy and infant care to qualifying pregnant women and infants.

  • Family PACT
    Provides no cost confidential family planning services and sexually transmitted infection services and treatment to qualifying clients during their childbearing years.

  • Medi-Cal
    No cost or share of cost coverage for qualifying families. Pick up an application and schedule an interview with Patient Registration in Arcata or Smith River.

    2. Scholarship Source
  • H.D. Timm Williams Scholarships are given annually to graduating High School students and Re-Entry or Continuing students in a health related field. Each student must maintain a 2.0 GPA. A High School student may receive two scholarships, one when graduating and another if they continue their education. The Re-entry or Continuing student may receive one scholarship. The following received scholarships for the School Year 03/04: High School: Six students; Re-Entry or Continuing: Six students.

    Mr. Williams was a UIHS Board Member who died in a traffic accident. He was the Chairman of the Board of Directors at the time of his death. His interest in our young people receiving a higher education prompted many in the community to donate to a fund set-up in his memory.

    Community Health and Wellness Financial Assistance Programs:

  • Community Support:
    The Community Health and Wellness Committee (C-HAWC) is committed to support activities and community functions in the UIHS Service Area that emphasize the importance of alcohol-free, tobacco-free, and drug-free activities in the community. To be able to support such activities and functions, the UIHS Community Health and Wellness Committee has engaged in various fund-raising activities to create the funds to support your community activities. The Committee is especially interested in youth and cultural activities.

  • UIHS Sports Mini-Grants:
    The Community Health and Wellness Committee (C-HAWC) is committed to support the activities of children in the UIHS Service Area. Young people may receive grants to cover the enrollment fees in any sports activity. The goal is promoting our youth to be active.
